After booking, check your email for an invitation to log in to your ChARM Patient Portal
Save your login information
You will receive an email with practitioner forms – please follow the instructions to fill out your questionnaires
Arrive 20 Minutes Early
for your first visit to the clinic
To clear up any questions, to sign any missing forms so that we can start your visit on time!
Cancellation Fees, Payment & Direct Billing Policies
If you have to cancel within 24 hours (or 1 business day) of the appointment start time, you will be charged the full visit fee.
NOTE: we are closed on Sunday, so all Monday cancellations must be made by Saturday!
Empower Health cannot guarantee that your insurance company will accept Direct Billing under all circumstances, so you will need to have a backup payment method.
